Keep this chart in mind as you’re coming up with a wellness vision, areas of focus, goals, and action steps. I recommend only having 1 or 2 areas of focus to keep in attainable, unless they somewhat overlap and you can commit to having more.

*Feel free to remove or add anything as needed. Re-read your wellness vision, areas of focus, and 3 Month Goals at least once per week. Use the “Action Planning” section daily.
My Wellness Vision
What is a Wellness Vision?
- A wellness vision describes your ideal future state of health and wellbeing. Your wellness vision does not need to be very long. However, it should be a good representation of the vision of who you are, how you see your future self and where you want to be. The wellness vision is later connected to the long-term goals you will set. It can be as simple as one to two sentences, one paragraph, or a page, or if you prefer, you can add a vision for different areas of focus from the grid wellbeing. The key is that it should inspire and motivate you to keep moving forward.
Developing a vision exercise:
You can do this exercise by doing a deep visualization or by writing down your answers on a sheet of paper.
- Take a deep breath, and slowly exhale.
- Let your mind move forward to one year in the future.
- Describe your ideal self. Describe your state of health and wellness. How do you feel? What changes did you make in your life? What helps you stay motivated? Who has helped you along the way?
- If you’d like to, repeat the previous step, picturing yourself five years from now.
For further reflection:
- How can your core values support your wellness vision?
- What challenges do you anticipate having to deal with in reaching your vision?
- What support system and resources do you currently have to help you accomplish your vision?
- How will achieving your wellness vision improve your life?

Now you need to define the gap, do this by identifying where you are now and where you want to be, then developing goals and action steps to help you get there.
Before you begin this, take a moment to reflect on this important question: What changes do you need to make to reach your goals?
